Our not-for-profit organization works to improve the total environment of older adults in the East Bay.
We support our participants’ medical, social, emotional and nutritional needs. Our care teams include 11 professional disciplines. Every week, they meet in one room to review our participants’ care plans and changes in medical conditions, social engagement and emotional well-being.
